Husband's Full Name
Please provide the full name, including any middle names, of the husband in the disolved marriage.
Wife's Full Name
Please provide the full name, including any middle names, of the wife in the disolved marriage.
Date of Marriage
Please provide the date of marriage. If the exact date is unknown, please give an indication of the year. This is used to assist a successful search in the event that there are multiple possible matches.
Divorce Court
Please indicate which court issued the original decree absolute. This is not mandatory, but does tend to speed up the issuing of replacement documents.
When was the divorce made absolute?
Whilst not essential, if you know the date or year of the divorce, please provide this.
Search Period
A search period of up to 10 years is provided with this service. Please indicate from what year you wish the search to be carried out. The search will then include 10 years from this year.
Additional 10-year Searches
Please indicate if you want a search to be carried out that exceeds 10 years. Choose the total number of years you need to be searched.
Case Number
If this is known, please provide the case number. This will usually reduce the time taken to deliver the replacement by a week or more.
Apostille Required

Scan and Send
Please indicate whether you would like a backup colour scan of the birth certificate emailed to you as it is dispatched. Please ensure that you include your email address when you complete your contact details during checkout.
Priority Service
Please indicate whether you require the certificate to be ready to dispatch in 4-5 weeks on our standard service,or approximately 7 days on our priority service. For clarification, working days do not include weekends or bank holidays.
